Job Summary
The Housing Navigator provides housing-focused case management and navigation services to individuals and families experiencing homelessness or at risk of homelessness. This position is responsible for conducting outreach, completing housing assessments, developing individualized housing plans, connecting clients with community resources, and assisting clients in obtaining and maintaining permanent housing. The Housing Navigator works collaboratively with landlords, service providers, and internal teams to remove barriers and support long-term housing stability.
Essential Duties & Responsibilities
- Conduct outreach and engagement with individuals and families experiencing or at risk of homelessness in shelters, encampments, hospitals, community organizations, and other outreach locations.
- Complete comprehensive housing assessments and identify client strengths, needs, and barriers to housing.
- Develop individualized housing plans and assist clients in achieving housing stability goals.
- Identify and secure housing opportunities by developing relationships with landlords, property managers, and community partners.
- Assist clients with housing applications, rental assistance programs, housing subsidies, and other financial resources.
- Coordinate supportive services, including referrals for healthcare, behavioral health, substance use treatment, employment, public benefits, and other community resources.
- Advocate for clients throughout the housing process and assist with landlord communication, lease requirements, and move-in coordination.
- Maintain accurate, timely, and complete client documentation in HMIS and other required databases while ensuring compliance with agency, county, state, and funding requirements.
- Collaborate with multidisciplinary teams and community partners to coordinate services and promote positive client outcomes.
- Participate in case conferences, team meetings, trainings, and community networking events.
- Maintain confidentiality and comply with all HIPAA, agency, and program policies.
- Perform other duties as assigned.
QualificationsEducation & Experience
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Administration, or a related field preferred.
- An equivalent combination of education and relevant experience may be considered.
- Experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, or other vulnerable populations is preferred.
- Knowledge of Housing First, Trauma-Informed Care, Harm Reduction, and client-centered service models is preferred.
- Experience with CalAIM, Medi-Cal programs, or housing navigation services is a plus.
Knowledge, Skills & Abilities
- Knowledge of community resources, affordable housing programs, and housing assistance services.
- Ability to build rapport with clients from diverse backgrounds using empathy and professionalism.
- Strong organizational, time management, and problem-solving skills.
- Excellent verbal and written communication skills.
- Ability to manage multiple priorities while meeting required deadlines.
- Experience using HMIS, Microsoft Office, and other electronic documentation systems.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Valid California driver's license, reliable transportation, and proof of automobile insurance may be required.
- Ability to travel locally to client appointments, outreach activities, meetings, and trainings.
