This is a field position meeting our members in their homes, hospitals and other facilities in the north Pima county area. Also a couple days a month you would travel to the Casa Grande and Arizona City area to visit members in facilities and home.
Description
At Catalytic Health Partners (CHP) our Clinical Liaisons are dedicated to meeting with members in their home focusing on assisting with medical education, attending appointments with members as needed to support their healthcare and social needs, transitional care for both physical and mental health admissions. CHP is an integrated population management company, and our Clinical Liaisons will work with the integrated teams to address the holistic needs of our members. We only take on members who are high cost, high risk and have low engagement with the health care community and our performance is based on how well we as a team address their complex holistic needs. We do not replace their community providers and clinics, but rather help guide members on how to effectively use their community resources. All our team is remote and, in the field, seeing members in their homes and in facilities as needed to ensure their safe transitions and skill development.
Area Covered
We are looking for someone to cover the north Pima county area. This includes Tucson, Marana, Oro Valley and the surrounding area. Also a couple days a month you would travel to the Casa Grande and Arizona City area to visit members in facilities and home.
Day to Day
Clinical Liaisons work to ensure Catalytic Health Partners and its teams are fully updated on members who are in medical/behavioral facilities and their current and planned treatments. Clinical Liaisons establish communication lines with the facilities to learn the reason for admission, current plan of care, metrics for improvement and planned discharge details. Additionally, Clinical Liaisons communicate with patients ensuring their needs are being take care of and they have clarity in the current care plan. In preparing patients for discharge a Clinical Liaison will work with community resources to ensure patients have the DME, medications, provider follow ups and at home social needs together before discharge. Clinical Liaisons work from an office setting to connect with all patients and facilities currently in the transitional care process. Then may go to field to meet visit member at the facility or in their home, if the field provider team is unable to within set timelines for proper transitional care. Once discharged Clinical Liaison is ensuring the start of care of any ordered post discharge community needs or scheduling follow up appointments with the members primary care provider and CHP healthcare providers within 7 days or sooner of discharge. Provides coordination of specialty referrals to required follow up provider outpatient visits, including attending outpatient visits with the member as needed.
Compensation
Compensation is made up of your base salary of $52,000 plus CHP pays for your medical, dental and a life insurance policy which equates to an additional $8500 in benefits not coming out of your pay. Additionally you have bonus opportunities where you may bonus up to an additional 20% of your salary per year.

Responsibilities
- Follow up with patients in medical and behavioral health facilities to understand care plans
- Start transitional care process
- Build communication lines between facility providers and CHP providers
- Ensure criteria for safe discharges are met before patients discharged from hospitals
- Ensure social, behavioral and medical needs are coordinated and in place before discharge of patient
- Obtain all documentation related to facility stay, transfers, and discharging
Skills
- Ability to read and interpret medical records

- Clear documentation skills
- Intermediate to advanced computer skills using MS Office products, Word, Excel, Access, Outlook, OneDrive, etc. as well as importing/exporting data to/from applications
- Knowledge of community resources
Minimum Requirements Education
Bachelor’s or master’s degree in healthcare related field with related experience OR
Is a graduate of medical school who is not currently working as a provider
