Key Responsibilities
OT Responsibilities
- Conduct comprehensive occupational therapy evaluations in patient homes.
- Develop individualized treatment plans focused on functional independence and safety.
- Provide therapeutic interventions including ADL training, home‑safety modifications, adaptive equipment recommendations, and energy‑conservation strategies.
- Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Educate patients and caregivers on safe mobility, equipment use, and daily‑living strategies.
Qualifications
OT Requirements
- Current Oregon Occupational Therapist license (or ability to obtain).
- New graduates and experienced clinicians welcome.
- Strong communication, assessment, and problem‑solving skills.
- Ability to work independently in a field‑based environment.
- Reliable transportation and valid driver’s license.
