The Registered Nurse/Care Manager assists clinicians in the development of the plan of care for patients. RNs are responsible for the care management and coordination of a patient panel and act as an advocate, educators, and resource persons for the respective population. RNs manage the logistical, financial, emotional, educational, and social barriers to care and compliance using the health-team model of care. Will plan, assess, and evaluate the patient's clinical status ensuring treatment plan and medication administration compliance through preplanning activities, care rounds, and patient engagement.

2+ years of experience as an RN in OBGYN, Internal Medicine, or community health setting

Required: Graduate of an accredited school of professional nursing by date of hire.

Preferred: BSN

Registered Nurse in the State of Maryland (or compact state)

Unexpired CPR certification from the American Heart Association or American Red Cross
