Job Summary
We are seeking a dedicated and energetic Home Health Physical Therapist to join our dynamic healthcare team. Responsibilities
- Conduct thorough assessments of patients’ physical conditions, including mobility, strength, and functional abilities, to inform individualized care plans.
- Develop and implement effective treatment plans that align with patient goals and clinical best practices, including physiotherapy techniques such as trigger point therapy.
- Provide hands-on therapy sessions focusing on patient rehabilitation, orthopedics, neurology, pediatrics, geriatrics, and other specialized areas.
- Collaborate closely with physicians, nurses, social workers, and other healthcare professionals to coordinate patient care and discharge planning.
- Monitor patient progress regularly; adjust treatment strategies as needed to optimize outcomes and ensure safety.
- Maintain accurate and detailed clinical documentation in accordance with medical terminology standards and electronic health records (EHR/EMR systems).
- Educate patients and their families on home exercise programs, safety precautions, and community resources to support ongoing recovery at home.
Qualifications
- Valid state licensure or certification as a Physical Therapist with practicing in a physical therapy role.
- Proven experience in home health settings or outpatient clinics with a focus on patient care across diverse populations.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards.
- Experience working with geriatric patients, pediatrics, orthopedics, neurology, and inpatient or hospital environments is highly desirable.
- Familiarity with outpatient rehabilitation center practices and outpatient clinic experience preferred.
- Proficiency in electronic health records (EHR) management systems such as EMR platforms used for documentation and treatment planning.
- Excellent client communication skills to effectively educate patients about their treatment plans and progress.
- Ability to work independently while collaborating effectively within multidisciplinary healthcare teams.
