Key Responsibilities:
- Collaborate to provide management and delivery of patient care plans.
- Support patients in Erie, PA and surrounding areas Travel.
- Ability to spend ~20% of your day driving to/from patient locations Schedule: Monday – Friday 8a-5p, 1 weekend a month, 2 holiday’s a year.
