Palm Point Behavioral Health is hiring a skilled Assessment Coordinator with a Master’s or Master’s in Social Work (MA/MSW) to join our dynamic team. Responsibilities
- Conduct comprehensive patient assessments including intake interviews, mental health evaluations, and risk assessments utilizing clinical tools and medical terminology.
- Coordinate with clinical staff to develop individualized treatment plans tailored to each patient’s needs, including group and individual therapy sessions.
- Manage crisis intervention procedures promptly and effectively, ensuring patient safety while adhering to HIPAA regulations and medical documentation standards.
- Collect and analyze data related to patient progress, treatment outcomes, and behavioral health trends for research purposes.
- Maintain accurate medical records, ensuring compliance with ICD-10/ICD-9 coding standards and CPT coding practices for billing purposes.
- Provide case management support by facilitating communication between patients, families, community resources, and healthcare providers.
- Apply evidence-based therapeutic techniques such as cognitive-behavioral therapy (CBT), motivational interviewing, applied behavior analysis (ABA), and trauma-informed care including PTSD management.
Requirements
- Master’s degree in Social Work (MSW); licensure preferred LCSW, LMFT, LMFTI, LMHC, or LMHCI, but not required.
- 1-3 years of experience required in behavioral health settings working with children, adolescents, and adults.
- Strong knowledge of mental health counseling techniques including grief counseling, addiction counseling, crisis management, and group therapy.
- Familiarity with medical terminology, HIPAA compliance, medical records management, and electronic health record systems.
- Experience in working within acute care environments such as hospitals or community clinics is highly desirable.
- Proficiency in data collection methods relevant to behavioral health research and case documentation practices.
- Ability to work effectively in high-pressure situations involving triage and crisis intervention while maintaining professionalism and compassion. The Intake Assessment Coordinator is responsible for being able to perform all aspects of Palm Point Behavioral Hospital’s intake and admission process for both Inpatient and Outpatient Servives, including efficient and effective handling of inquiry calls, knowledge of and communication with referral sources; providing hospital and program information to individuals, agencies, facilities, and professionals; making outside referrals, when appropriate; assessing and triaging an individual’s mental health status; making patient safety and level of care recommendations to the PPBH physician; identifying insurance benefits, pre-certification of patient care; completing all admission requirements; communicating patient safety concerns and pertinent patient information to unit nursing staff and documenting all necessary information in Med Series 4.

- Knows PPBH programs and is able to answer general program information
- Determines initial clinical and financial eligbility
- Conducts psychosocial evaluation on each assigned admission, gathers data
- Coordinates with the treatment team, family and patients
