What You’ll Do:
- Conduct patient assessments & create care plans
- Perform OASIS assessments & monitor progress
- Coordinate care, referrals & discharge planning
- Work closely with patients and families
What You Need:
Active CT RN License + CPR certification
1+ year RN experience (home care a plus)
Valid driver’s license & reliable vehicle
