Up to $70/hr | Full-Time | Onsite | Contract opportunity with potential to convert
We are hiring a Medical Social Worker, also referred to as a Social Work Clinician, for a full-time onsite position in Palo Alto, CA.
In this role, you will provide psychosocial support to patients and their families as they navigate illness, treatment, changes in functioning, and transitions in care. You will work alongside an interdisciplinary healthcare team to assess needs, address barriers to care, and connect patients with appropriate resources and services.

What You’ll Do
- Conduct psychosocial assessments of patients and families
- Identify social, emotional, cultural, financial, and environmental barriers to care
- Develop and implement psychosocial treatment plans with the healthcare team
- Provide supportive counseling, grief counseling, crisis intervention, and psychoeducation
- Help patients and families understand and adapt to diagnoses, treatment plans, and changes in care needs
- Coordinate referrals to community resources, specialty services, entitlement programs, and other support systems
- Manage an assigned caseload and identify patients who may benefit from social work services
- Participate in interdisciplinary rounds, case conferences, and care planning
- Maintain accurate electronic medical records, progress notes, reports, and handoffs
- Contribute to quality improvement efforts involving social work services and patient care
What You’ll Bring
- Master’s degree in Social Work from a CSWE-accredited college or university
- Knowledge of medical social work practices and healthcare delivery systems
- Ability to assess patient and family needs and develop appropriate interventions
- Experience with counseling, crisis intervention, case management, and care coordination
- Strong written documentation and verbal communication skills
- Ability to manage confidential information and respond appropriately in stressful situations
- Understanding of cultural considerations, family dynamics, child development, illness, injury, and loss
