Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and mobility support to patients of all ages.
- Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders.
- Monitor and record patient vital signs, observe changes in health status, and report findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Support patients with mobility transfers using patient transfer methods and equipment like Hoyer lifts or gait belts while ensuring safety protocols are followed.
- Administer medications as prescribed in accordance with home healthcare regulations and document all medical documentation meticulously.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment for clients.
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavior management strategies.
- Support clients during end-of-life or hospice care with compassion and dignity while respecting patient confidentiality (HIPAA compliance).
- Use patient observation skills to detect changes in condition or behavior that may require intervention or escalation of care.
- Communicate effectively with clients and their families to ensure comfort, understanding, and trust throughout the caregiving process.
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred.
- Demonstrated experience working with individuals with developmental disabilities or geriatrics is a strong asset.
- Knowledge of medical terminology, catheter care, wound care, and CPR/First Aid certification is required.
- Experience using EMR/EHR systems for documentation and client communication is desirable.
- Familiarity with home health care assistance techniques including patient transfer methods and medication administration is essential.
- Previous live-in employment experience or working within hospice or memory care settings will be considered advantageous.
- Ability to lift heavy objects safely and perform physically demanding tasks while maintaining patient safety standards.
