We are seeking a Registered Nurse (RN) to join our team and provide clinical support for our Group Adult Foster Care (GAFC) program.
Responsibilities:
- Complete member assessments and care plans
- Conduct admissions, annual evaluations, and follow-up visits
- Collaborate with care managers and program staff
- Ensure compliance with MassHealth GAFC regulations
- Provide health education and support to members
Qualifications:
- Preferred Bilingual (Spanish)
- Current Massachusetts RN license
- CPR license
- Experience with GAFC, home health, or long-term care preferred
- Strong organizational and documentation skills
- Compassionate, reliable, and able to work independently
