Title: RN Patient Care Coordinator
Location: South San Francisco, CA 94080
Duration: 13 weeks/40 hours
Pay: $2800 - $3200 per week
Shift: 11:00 PM to 07:30 AM
Requirements:
- 2 years of recent acute care RN experience required.
- Previous Patient Care Coordination, Care Management, Case Management, or Utilization Management experience required.
- Must have BLS (AHA) & CA state license
- EPIC experience required.
