We are seeking Full time RN FIELD SUPERVISORS in the following locations- Queens, Bronx and Manhattan. As a field supervisor, you will further Bestcare’s goal for our patients to remain safely in their homes, functioning at their optimum level of independence.
Job Responsibilities:
- Conduct initial home visits to assess patient needs, develop the Aide Plan of Care, and provide ongoing oversight and support
- Communicate with a patient’s physician, insurance, and other team members to ensure delivery of appropriate patient care
- Supervise patient care provided by our Home Health and Personal Care Aides
- Participate in orientation, teaching, and evaluations of care providers
- Responsible for the review and maintenance of patient records
Job Qualifications:
- Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience
- Must have a valid driver’s license and transportation for the position in the Bronx
- Previous home care experience is a plus
- Knowledge of EMR HHAX a plus
- Bilingual English Spanish required for Manhattan and Bronx locations
Pay: From $75,000.00 per year
