Job Title: Admit Nurse, RN

Job Overview:
As an admission nurse, you'll conduct comprehensive patient assessments, facilitate seamless admission procedures, and deliver compassionate care coordination, ensuring the best support for hospice patients and their families.
Responsibilities:

Coordinates and oversees all aspects of new patient admissions

Collaborates with patient care teams to ensure seamless communication

Educates families on patient condition, needs, and care

Performs head-to-toe physical assessments

Timely and accurately completes documentation

Other duties as assigned
Requirements:
Education:
Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure:
Current State license as a Registered Nurse; Current driver’s license. Current CPR Certification.
Experience:
Two (2) years' experience as a Registered Nurse in a clinical care setting and one (1) year full-time hospice.
Skills:
Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.
Good interpersonal skills.
Transportation:
Reliable transportation and valid auto liability insurance.
