We are seeking compassionate, dependable, and clinically skilled Registered Nurses to provide quality care to patients in the comfort of their homes. As a Home Health RN, you will be responsible for assessing patient needs, developing individualized plans of care, coordinating services with the interdisciplinary team, and ensuring compliance with Medicare, AHCA, and agency regulations.
Responsibilities
- Perform comprehensive patient assessments and OASIS documentation.
- Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Supervisory, Routine, and Discharge Visits.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Administer medications, treatments, wound care, IV therapy, and other skilled nursing services as ordered.
- Monitor patient progress and communicate significant changes in condition to the physician and clinical management team.
- Provide education to patients and caregivers regarding disease management, medications, safety, and treatment plans.
- Coordinate care with physicians, therapists, social workers, aides, and other healthcare professionals.
- Supervise and evaluate Home Health Aides and other assigned personnel in accordance with agency policy.
- Maintain accurate and timely clinical documentation in compliance with Medicare and agency requirements.
- Participate in quality improvement initiatives and agency in-service education programs.
Qualifications
- Graduate of an accredited School of Nursing.
- Active Florida Registered Nurse (RN) License in good standing.
- Current BLS/CPR Certification.
- Minimum of one (1) year experience in one of the following:
- Acute Care
- Home Health
- Community/Public Health
- Outpatient Clinical Setting
- Previous Home Health and OASIS experience preferred.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while managing a flexible field schedule.
AHCA Requirements
Applicants must meet all AHCA and Medicare employment requirements, including:
- Eligible to work for a Florida AHCA-licensed Home Health Agency.
- Successful completion of AHCA Level 2 Background Screening through the Florida Care Provider Background Screening Clearinghouse.
- Current physical examination and TB screening.
- Valid Florida Driver's License.
- Reliable transportation.
- Proof of current automobile insurance.
- Ability to travel throughout assigned territory.
- Compliance with all agencies, AHCA, Medicare, OSHA, and HIPAA regulations.
