Oversee the provision of medically related social service needs. (i.e., to assist the resident in maintaining or improving the ability to manage everyday physical, mental, and psychological needs) Ability to:
Assess the resident and identify and seek ways to support the resident's individual needs and preferences, customary routines, concerns and choices. Assist the resident and his or her family or responsible party to adjust to life at Daughters of Miriam.
Maintain contact with family members (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
Assist with financial and legal matters, e.g. applying for pensions and other benefits, Medicaid and Medicare, advance directives, referrals to lawyers and referrals to funeral homes for preplanning arrangements. Coordinate financial arrangements with the Home's Business Office.
Act as a liaison with social, health and community agencies and be responsible for referrals of residents when appropriate. Ensure documentation in the resident's record concerning the results of such referrals.
Assist the resident and/or staff in obtaining needed adaptive equipment, clothing and personal items.
Coordinate family and community resources as needed to solve financial needs and to promote emotional security.
Coordinate and participate in family and resident activities designed to promote social interaction, reality orientation, and intellectual stimulation.
Assist in the organization or community events related to resident's needs and interests.
Plan, schedule and implement room visits for residents who do not leave their rooms.
