We are looking for a Care Coordinator who has empathy, patience, communication skills and leadership abilities working with families. We need a person who is energitic and willing to learn the ins and outs of our public resources. In this vital role, the Care Coordinator will serve as a compassionate liaison between families and their resources to a self sustainable life. Your energetic approach and attention to detail will help create a positive experience for every family you assist, fostering trust and promoting successful outcomes.
Responsibilities
- Assist in maintaining office records, documents, and data necessary for ongoing operation accountability of the Family Promise of Wayne County NY Inc Affiliate.
- Provide Case Management services to families in need of assistance through the ROTH program.
- Maintain regular follow up with current and prior families.
- Maintain a professional demeanor during all interactions, demonstrating excellent communication skills while managing sensitive information
- Proficiency in Microsoft Office applications (Word, Excel, Outlook), Family Link and Google Suite for documentation and communication purposes.
- Dedicate oneself to maintaining the highest levels of confidentiality for every family we provide services to.
- Knowledge of HMIS system or willingness to learn.
- Ability to document geographic information to assist our organization in grants
- Excellent phone communication skills coupled with the ability to handle interactions professionally.
- Ability to maintain professional boundaries with every family
- Willingness to work as a Team but also capable of working independently
- Need the capability to build rapport with families from diverse backgrounds
- Effectively network with various community agencies, identifying resources, to meet the individualized needs of each family
- Work cooperatively with the Executive Director and Administrative Assistant to ensure good working relationship, clear division of duties, and effective service to the families
- Candidate must be able to sit for long periods of time, have a valid NY State Driver's license, ability to lift 25 lbs (donations we might receive), have excellent organizational skills of donated itemsand can walk up and down flights of stairs.
Pay: $18.00 per hour
Work Location: In person
