As a Licensed Practical Nurse (LPN) or Graduate Practical Nurse (GPN) on our Chronic Care team, you'll provide compassionate, patient-centered care to individuals with complex, long-term medical needs. Working under the supervision of a Registered Nurse, you'll collaborate with the interdisciplinary team to assess patient needs, implement individualized care plans, monitor patient progress, and support positive clinical outcomes. At HFSC, you'll build lasting relationships with patients while gaining opportunities to expand your skills and explore career growth across our specialty programs.
Qualifications:
Required: Graduate of an accredited licensed practical nurse program
Required: Current licensure as a licensed practical nurse in the State of Connecticut
Preferred: Valid/Current CPR certification -required within three months of employment
Preferred: Minimum one year LPN experience in a medical/surgical or long-term care setting.
