Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs and reporting any changes to Nurse Supervisor
- Prepare nutritious meals tailored to dietary needs and assist with feeding as necessary
- Support medication administration in accordance with care plans and HIPAA regulations to ensure patient safety
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Implement personalized care plans designed by healthcare teams, ensuring each client’s unique needs are met with compassion and professionalism
Requirements
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Knowledge of patient observation techniques, dementia care (including Alzheimer’s), memory care, and hospice care practices
- Familiarity with HIPAA regulations to ensure confidentiality of patient information
- Ability to perform CPR and First Aid certifications (or willingness to obtain upon hire)
- Strong skills in assisting with ADLs, medication management, patient monitoring, and manual handling techniques
- Experience supporting individuals with developmental disabilities, neurodevelopmental disorders such as autism, intellectual disabilities, or geriatrics is a plus
- Excellent communication skills combined with a caring attitude to build trusting relationships with clients and their families
