Position Overview
Abound Rehabilitation Services is seeking compassionate, reliable, and experienced Direct Care Workers (DCW) to join our team in Wayne County. In this role, you will provide essential daily support, personal care, and community integration assistance to individuals within our residential settings.
We are looking for dedicated professionals who are passionate about empowering individuals, fostering independence, and maintaining a safe, supportive living environment in compliance with Wayne County Mental Health/PIHP standards and State of Michigan regulations.
Key Responsibilities
· Personal Care & Daily Living Assistance: Assist residents with activities of daily living (ADLs), including personal hygiene, bathing, grooming, dressing, toileting, and mobility assistance.
· Health & Medication Support: Administer medications in accordance with physician orders and state/county guidelines (upon completion of required training); monitor and report changes in physical or behavioral health.
· Meal Preparation & Housekeeping: Plan and prepare nutritious meals according to dietary guidelines, perform light housekeeping, laundry, and maintain a clean, safe, and sanitary living environment.
· Documentation: Complete accurate, timely, and detailed shift logs, incident reports, progress notes, and medication administration records (MARs) in accordance with Wayne County standards.
· Community Integration & Engagement: Support residents in social, recreational, and skill-building activities both within the home and out in the community.
· Safety & Crisis Prevention: Implement individualized behavior support plans, maintain resident safety, and apply approved crisis prevention and intervention techniques as needed.
Required Qualifications & Experience
· Prior Experience: Minimum of one (1) year of prior experience as a Direct Care Worker or Direct Support Professional (DSP) is required and/or current nursing students are strongly encouraged to apply. Candidates enrolled in an accredited nursing program will be given strong consideration, even if they do not have prior direct care experience.
· Preferred Background: Special consideration and strong preference will be given to candidates with prior experience in:
o Nursing Homes / Long-Term Care Facilities
o Group Homes / Adult Foster Care (AFC) Facilities
o Assisted Living or Similar Caregiver Environments
· Education: High School Diploma or GED equivalent.
· Driver’s License: Valid Michigan Driver’s License with a clean driving record (reliable transportation required).
· Background Checks: Must successfully pass all required Michigan State Police criminal background checks, Central Registry clearances, and drug screenings as mandated by Wayne County and State regulations.
· Preferred Background: Special consideration and strong preference will be given to candidates with experience in:
o Nursing Homes / Long-Term Care Facilities
o Group Homes / Adult Foster Care (AFC) Facilities
o Assisted Living Facilities
o Behavioral Health, Mental Health, or Substance Use Disorder treatment settings
o Hospital or Rehabilitation Facilities
Certifications & Training Requirements
· CPR and First Aid Certification (or willingness to complete upon hire).
· Completion of Wayne County / Detroit Wayne Integrated Health Network (DWIHN) approved Direct Care Worker training modules (e.g., Bloodborne Pathogens, Rights of Individuals Received Services, Medication Administration) preferred or required within 90 days of hire.

Basic computer literacy and proficiency with electronic documentation systems, including the ability to accurately complete electronic progress notes, incident reports, medication administration records (MARs), email communication, and other required documentation.
Ability to learn and navigate Electronic Health Record (EHR) systems and other agency software.
Ability to lift, transfer, and assist residents safely (ability to lift up to 50 lbs.).
Reliability, punctuality, strong organizational skills, and attention to detail.
Ability to work independently while collaborating effectively as part of a multidisciplinary team.
Strong problem-solving skills with the ability to respond appropriately in emergency and crisis situations.
Commitment to maintaining resident confidentiality and complying with HIPAA, Wayne County/DWIHN requirements, and State of Michigan regulations.
Compensation & Benefits
· Starting Rate: $17.00 per hour
o After 90 days of work there will be a merit review with possible 50 cent increase
