Job Summary
Orange Del Sol ADHC are seeking a compassionate, organized, and bilingual Medical Social Worker to join our Adult Day Health Care program in Tustin, California.
The Social Worker is responsible for providing comprehensive psychosocial services, including assessments, care planning, case management, counseling, and coordination of community resources for frail older adults and adults with disabilities.
As a key member of the multidisciplinary team (MDT), the Social Worker ensures the delivery of person-centered, culturally competent services that promote participants’ emotional well-being, independence, safety, dignity, and quality of life in a structured day program setting.
Services are provided under the scope of practice of a Medical Social Worker and in accordance with applicable supervision requirements when necessary.
Responsibilities
- Conduct comprehensive psychosocial assessments, reassessments, and participate in individualized care plan development in accordance with CBAS/ADHC and California Title 22 regulatory requirements
- Provide individual, family, and caregiver counseling, emotional support, and psychosocial interventions within scope of practice
- Develop, implement, and monitor individualized care plans in collaboration with the multidisciplinary team (MDT)
- Coordinate community resources, Medi-Cal benefits, long-term care services, and supportive programs for participants and families
- Provide ongoing case management, including referrals, advocacy, follow-up services, and discharge planning
- Assist with participant intake, enrollment, annual reassessments, care transitions, and discharge processes
- Monitor participants’ psychosocial status and promptly report changes, concerns, or risks to the multidisciplinary team
- Facilitate participant and family education regarding available community resources and supportive services
- Actively participate in multidisciplinary team (MDT) meetings, care conferences, and family meetings
- Maintain accurate, timely, and confidential documentation in compliance with HIPAA, CDPH, and Title 22 regulations
- Ensure compliance with participant rights, confidentiality, and California Title 22 regulations
- Collaborate with nursing staff, activity staff, rehabilitation therapists, dietitians, caregivers, and external agencies to ensure coordinated care
- Advocate for participant rights, dignity, self-determination, and culturally sensitive care
- Comply with mandatory reporting requirements, including elder abuse, neglect, and exploitation reporting laws
- Perform other related duties as assigned within scope of practice
Qualifications
- Master’s Degree in Social Work (MSW) from a CSWE-accredited program preferred.
- OR
- Master’s Degree
- in related fields such as Psychology, Gerontology, or Counseling with
- at least one (1) year of experience
- providing social services in aging, health, or long-term care settings.
- Bachelor’s Degree in Social Work (BSW) with at least two (2) years of experience providing social services in aging, health, or long-term care settings.
- Bilingual in English and Chinese required
- Strong knowledge of psychosocial assessment, counseling, case management, and care coordination in community-based settings
- Familiarity with Medi-Cal programs, community resources, long-term care services, and California title 22 regulations preferred
- Experience working with frail older adults and adults with disabilities preferred
- Strong communication, interpersonal, organizational, and documentation skills
- Ability to work effectively in a fast-paced multidisciplinary team (MDT) environment
- Demonstrated cultural competence and sensitivity when working with diverse populations
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Must meet all California Department of Public Health (CDPH) requirements, including background clearance, TB testing, and health screening
- Must comply with all mandatory reporting laws, including elder abuse, neglect, and exploitation reporting requirements
Additional Compliance & Employment Terms
- Services are performed within the scope of a medical social worker practice and applicable supervision guidelines
- This position does not require independent clinical diagnosis or psychotherapy unless appropriately licensed
- Active participation in multidisciplinary team (MDT) meetings is required
- Documentation must be completed accurately and in a timely manner in accordance with program and regulatory standards
- Compensation is based on experience and qualifications
- Work schedule may vary based on program operational needs
