HomeNP is expanding our mobile healthcare provider network and is actively building a talent pool for future opportunities across Michigan, including:
- Detroit - Serving Detroit, Warren, Dearborn, Southfield, Livonia, Sterling Heights, Troy, Royal Oak, and surrounding communities.
- Flint -Serving Flint, Grand Blanc, Burton, Fenton, Davison, Swartz Creek, Clio, Flushing, and surrounding communities.
- South Kalamazoo - Serving Kalamazoo, Portage, Battle Creek, Paw Paw, Plainwell, and surrounding communities.
Pay varies based on role, experience, and market
About the Role
Our providers deliver comprehensive primary care services directly to patients in:
- Private residences
- Assisted living facilities
- Independent living communities
- Other residential care settings
This innovative care model improves continuity of care while reducing transportation and access barriers for older adults.
Providers work independently in the field while receiving support from centralized scheduling, remote medical assistants, and administrative teams.
Typical Responsibilities
- Conduct wellness visits and routine health assessments
- Diagnose and manage acute and chronic medical conditions
- Review, prescribe, and manage medications as appropriate
- Coordinate care with specialists, caregivers, and facility staff
- Support preventive healthcare initiatives
- Educate patients and families regarding treatment plans and disease management
- Document patient encounters accurately and timely
- Deliver compassionate, patient-centered care in home-based settings
Preferred Experience
Experience in one or more of the following areas is strongly preferred:
- Geriatrics
- Primary Care
- Home Health
- Chronic Disease Management
- Palliative Care
- Hospice Care
- Urgent Care
- Wound Care
Qualifications
Nurse Practitioner (NP)
- Active Michigan Nurse Practitioner license
- MSN or DNP degree
- Current national board certification
- DEA registration (or ability to obtain)
Physician Assistant (PA)
- Active Michigan Physician Assistant license
- NCCPA certification preferred
- DEA registration (or ability to obtain)
What to Expect
- Providers typically cover a service area extending approximately 45 miles from their residence, with occasional travel beyond that range based on operational needs.
- Flexible scheduling options may be available depending on territory needs.
- Providers receive operational support to help maximize patient care time while minimizing administrative burden.
