We are now hiring PER Diem Therapist to work within our skilled nursing facility as part of our rehabilitative department: Physical Therapist, Occupational Therapist Registered (OTR), and Speech Language Pathologist (SLP). We are committed to providing excellent rehabilitation services to our residents, whether the need is short term rehab, skilled nursing care, long term or hospice care. The therapist will provide evaluation and therapeutic services as needed by our patients in addition to the following:
- Plan, develop, organize, implement and evaluate direct therapy in accordance with current rules regulations and guidelines that govern long term care facilities.
- Participate in discharge planning, implementation and development of residence care plans, and residence assessments.
- The Therapist will also provide supervision and guidance to COTA'S, PTA's and rehabilitation aides.
- Helps educate family and staff to optimize patient outcomes and safety.

Requirements:
- Bachelor's degree required
- Certified FL license in Occupational, Speech, Physical Therapy
- Excellent communication skills
- Great clinical, personal, documentation and implementation skills
- Skilled nursing facility (SNF) experience preferred
- This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse.
