Overview
Our home-based wound care program is seeking a Mobile Wound Care Nurse Practitioner to join our growing team. This territory is centered on Tinley Park, IL and providers are kept within a 50-minute patient travel radius, so your day is spent delivering care rather than driving long distances. This structured, compact coverage area makes the role a great fit for clinicians relocating from larger metro areas or commuting in from nearby communities.
Responsibilities
- Conduct in-home wound assessments and develop individualized treatment plans
- Perform wound debridement, dressing changes, and other advanced wound care procedures within scope of practice
- Collaborate with primary care providers, home health agencies, and specialists to coordinate patient care
- Document visits accurately and in a timely manner using our EMR system
- Educate patients and caregivers on wound care management and prevention
Qualifications
- Active Nurse Practitioner license in Illinosi
- Certification in wound care preferred (or willingness to obtain)
- Experience with home health or mobile care delivery a plus
- Reliable transportation and a valid driver's license
- Strong clinical judgement and comfort working independently in the field
