Compensation: Approximately $22.00 - $27.00 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
Functions as a liaison and advocate for patients between their care team, specialty care, and/or other relevant staff and assists patients in navigating referrals to community resources and specialty care.

- Bachelor's in Psychology, Social Work, Counseling or related degree.
- 5 years of experience can be substituted for a degree.
- Collaborative, and client centered.
- Organization is necessary to managing client information, appointments and follow up items.
- Collaborative team member who is ready to jump in, support our clients and team.
- A 40 hrs. per week role and with benefits (medical, dental, vision, life, disability and retirement plan).
Knowledge, Skills and Abilities:
- Bilingual in Spanish required.
- Sensitivity to under resourced individuals and communities.
- Case management for complex patients.
