Admissions Nurse
Job Summary:
Stroudsburg Post Acute Nursing and Rehabilitation is seeking a dedicated and compassionate Admissions Nurse to join our team! As an Admissions Nurse, you will play a crucial role in ensuring a smooth transition for our patients from the hospital to our post-acute care facility. This is a full-time position that offers a dynamic and rewarding work environment.
Responsibilities:
Conduct thorough patient assessments and admit patients to our facility in accordance with regulatory guidelines
Develop and implement individualized care plans in collaboration with our interdisciplinary team
Communicate effectively with patients, families, and healthcare providers to ensure a seamless transfer of care
Monitor patient progress and make adjustments to care plans as needed
Collaborate with our quality improvement team to identify areas for improvement and implement changes
Requirements:
Current LPN or RN license in the state of Pennsylvania
Strong assessment and clinical skills
Excellent communication and interpersonal skills
Ability to work effectively in a fast-paced environment
