Case Manager-Crisis Response Services
Description:
As a Case Manager in Crisis Response Services (CRS), you will play a vital role in supporting adults experiencing mental health challenges as they navigate periods of crisis and transition. Through outreach, assessment, care coordination, and connection to community resources, you will help individuals access the services and supports needed to achieve stability, recovery, and improved quality of life.
This is a dynamic position where no two days are the same. You will have the opportunity to work independently while receiving ongoing support from experienced supervisors and collaborating with a multidisciplinary team of clinical and community-based professionals. This position involves extensive coordination of behavioral health, medical, substance use, and community-based services.

What You’ll Do Day-to-Day
- Conduct outreach and engage individuals in crisis with empathy and professionalism
- Complete assessments to identify immediate needs, risks, and strengths
- Develop and implement individualized service plans focused on stabilization and recovery
- Coordinate care across behavioral health, medical, and social service providers
- Connect individuals to housing, treatment, benefits, and other community resources
- Maintain accurate and timely documentation in accordance with program requirements
- Collaborate regularly with internal teams and external partners to ensure continuity of care
- Provide ongoing follow-up and support to promote stability and prevent future crises
Schedule:
Monday – Friday 8:30 a.m. – 5:00 p.m. (one day scheduled 10:30 a.m-7:00 p.m.- to be determined based on program need).
Use of agency vehicles as well as use of own car (mileage and reimbursement).
Minimum Requirements:
Associate’s Degree in relevant human services field plus two years of relevant experience or Bachelor’s Degree in relevant human services field.
Salary Range:
Salary/Hourly Range: $19.00- $21.50 per hour depending on experience
Bilingual Spanish/English Fluency: $19.96- $22.59 per hour depending on experience

Work Location: This role requires a hands-on presence to deliver the best care possible. You will work directly with clients on-site and in the local community. Regular travel throughout the community is required.
