Location: California 93405
Pay: $2,743 – $2,905/week
Schedule: Day Shift, 4x10s (no set days; occasional weekends)
Hours: 7:30 AM – 6:00 PM
Call: None
Perform utilization review using InterQual criteria
Coordinate discharge planning with patients, families, and agencies
Collaborate with providers to ensure appropriate level of care
Monitor patient progress and support timely care transitions
Document case management activities and authorizations
Education & Experience
Associate Degree (Required)
BSN (Preferred)
3–5 years acute care nursing experience (Required)
At least 1 year hospital case management experience (Required)
Utilization review/discharge planning experience (Preferred)
Licensure & Certifications
Skills
Knowledge of InterQual criteria and Medicare guidelines (Preferred)
Proficiency in Word, Excel, Outlook, and basic computer systems
