Job Overview
We are seeking a dedicated and compassionate Long Term Care Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities
- Administer medications, including injections and IV infusions, following prescribed protocols and maintaining accurate documentation.
- Conduct comprehensive patient assessments, monitor vital signs, and evaluate clinical conditions to inform care plans.
- Assist with activities of daily living, including mobility support, catheter care, tube feeding, and wound management.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate individualized care strategies.
- Maintain strict adherence to HIPAA regulations and clinical confidentiality policies during all patient interactions and documentation.
- Utilize electronic health record (EHR) systems such as Epic or Cerner for accurate medical documentation and communication.
- Provide compassionate dementia care, memory support, and specialized assistance for residents with Alzheimer's disease or other cognitive impairments.
- Support discharge planning, case management activities, and family communication to ensure continuity of care.
- Follow long-term care regulations and standards to uphold quality assurance and safety protocols within skilled nursing facilities or assisted living environments.
Qualifications
- Valid LPN license with current state licensure required; additional certifications such as CPR and Basic Life Support (BLS) are essential.
- Proven experience working in long-term care facilities, nursing homes, or assisted living communities.
- Critical care experience or hospital experience preferred; familiarity with ICU or emergency medicine is advantageous.
- Knowledge of ICD-9/ICD-10 coding systems for accurate medical documentation and billing processes.
- Experience working with individuals with developmental disabilities or behavioral health needs is highly desirable.
- Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth for efficient patient record management.
- Strong understanding of anatomy, physiology, medication administration protocols, and infection control practices.
- Excellent communication skills to effectively interact with residents, families, and interdisciplinary team members.
- Ability to handle acute pain management cases and participate in triage activities confidently.
