CSL Staffing is hiring a Community Support Outreach Specialist for a growing behavioral and mental health healthcare company in San Diego. Position: Community Support Outreach Specialist
Location: San Diego, CA 92123
Pay Rate: $25–$26/hour
Schedule: Full-Time, Monday–Friday, 8:30 AM – 5:00 PM | Temp-to-Hire
What You'll Do:
- Conduct in-depth assessments over the phone with individuals and families to understand their housing, financial, health, and employment needs
- Help members develop and follow personalized housing plans to overcome barriers and achieve stable housing
- Provide culturally sensitive, one-on-one case management tailored to each person's needs
- Support clients with life skills, job readiness, and budgeting to build self-sufficiency
- Serve as a liaison between tenants and landlords to resolve issues and prevent evictions
- Conduct home visits after housing placement to ensure continued stability
- Coordinate with service providers and attend team meetings to deliver connected care
- Maintain detailed records and provide weekly and monthly updates to supervisors
- Support housing transition, sustainability, nursing facility-to-home transitions, post-hospital housing, and housing navigation services
- Conduct extensive phone outreach to members, community partners, and service providers to coordinate care and maintain consistent communication
- Perform in-person client visits once training is completed
What You Need:
Education & Experience:
- High school diploma or GED required
- At least 1 year of experience in case management, healthcare, social services, or customer service
Skills & Abilities:
- Valid California driver's license and car insurance required (mileage reimbursed)
- Strong communication, organization, and active listening skills
- Comfortable using computers for documentation and communication
- Able to work independently and as part of a team
- Able to multitask and manage multiple clients effectively
- Compassionate, patient, and able to handle emotionally challenging situations
- Strong problem-solving skills under pressure
- Bilingual skills, especially in Arabic, are a strong advantage
- Able to sit for extended periods and lift up to 15 lbs
