Campbell Health Solutions is a thriving, physician-owned medical practice seeking two experienced Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to join our growing team.
We are a busy, family-owned practice specializing in Family Medicine and Pain Management, where patient relationships, teamwork, and quality care come first. Our providers enjoy the support of an experienced clinical and administrative staff, allowing them to focus on delivering exceptional patient care.
We are seeking compassionate, motivated, and confident providers who are comfortable practicing with a high degree of autonomy while collaborating closely with our physicians. This is an excellent opportunity to join a well-established practice with a loyal patient base and continued growth.
Responsibilities
- Evaluate, diagnose, and treat patients across the lifespan.
- Manage acute and chronic medical conditions.
- Perform annual wellness visits, preventive care, and routine follow-up appointments.
- Order and interpret diagnostic testing.
- Develop and manage treatment plans.
- Prescribe medications in accordance with state and federal regulations.
- Provide patient education and promote preventive healthcare.
- Collaborate with physicians and clinical staff to ensure outstanding patient outcomes.
- Maintain timely and accurate documentation within the electronic medical record.
- Conduct thorough patient assessments, including medical histories and physical examinations, to establish accurate diagnoses.
- Develop and implement individualized treatment plans.
- Manage medication prescribing in accordance with HIPAA regulations, DEA guidelines, and state laws.
- Utilize EMR/EHR systems such as Epic for accurate medical documentation and medical records management.
- Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes.
Qualifications
- Current Illinois NP or PA license.
- DEA license (or ability to obtain).
- Board certification.
- Minimum of 2 years of clinical experience preferred in Family Medicine, Primary Care, Urgent Care, Internal Medicine, or Pain Management.
- Strong clinical judgment and excellent communication skills.
- Ability to thrive in a fast-paced, team-oriented environment.

Pay: $100,000.00 - $130,000.00 per year
