Schedule: Typical hours are Monday–Friday, 8:30 AM–5:00 PM, with an estimated 4–5 patient visits per day—and the flexibility to adjust your visit times to fit your preferences and patient schedules.
What You’ll Do
- Perform in-home evaluations and develop individualized treatment plans.
- Deliver evidence-based occupational therapy to help patients regain independence and improve daily living skills.
- Educate patients and caregivers on adaptive strategies, assistive devices, and home modifications.
- Collaborate with nurses, PTs, and other team members to ensure coordinated care.
- Maintain accurate, timely documentation in compliance with home health standards.

What We’re Looking For
- Licensed Occupational Therapist (OTR/L) in the state of Michigan.
- Home health experience preferred, but not required.
- Strong clinical skills and a commitment to patient-centered care.
- Willingness to travel to patients’ homes throughout our service area (approximately a 45-mile radius from Gaylord).
- Positive, team-oriented attitude—we support each other and do what’s best for patients.
Job Types: Full-time, Part-time
Pay: $95,000.00 per year
