Occupational Therapist – Home Health
Partners In Home Care (PIHC), Western Montana’s leading non-profit home health provider since 1987, is seeking a dedicated and compassionate Occupational Therapist to join our Home Health team. This role offers the opportunity to provide personalized, one-on-one care in patients’ homes, helping individuals regain independence and improve quality of life through skilled, patient-centered therapy.

Department: Home Health
Employment Type: Full-Time or Part-Time
Schedule: Monday–Friday, with flexibility based on FTE and patient care needs
Location: Based out of Missoula County, Ravalli County, or Lake County but may require travel to other counties within our service area.

Conduct comprehensive in-home occupational therapy evaluations and develop individualized treatment plans

Provide skilled, evidence-based occupational therapy interventions and adjust plans as patient needs evolve

Complete and score OASIS assessments accurately and in compliance with Medicare regulations

Maintain knowledge of payer guidelines and Medicare Conditions of Participation

Participate in interdisciplinary case conferences and QAPI initiatives

Supervise Occupational Therapy Assistants (OTAs) and provide oversight to CNAs when applicable

Manage patient care episodes including admissions, reassessments, and discharges
-
Document services and communications accurately using point-of-care electronic systems

Coordinate daily schedules to support continuity of care and efficient travel

Maintain active Montana OT licensure, required continuing education, and ethical professional standards

Participate in agency training and professional development activities
REQUIREMENTS

Bachelor’s degree or higher from an accredited Occupational Therapy program

Current Occupational Therapist license in the State of Montana (or ability to obtain)

Valid driver’s license, reliable transportation, and appropriate auto insurance

Access to high-speed internet

Two or more years of occupational therapy experience preferred

Home health or clinical rehabilitation experience a plus
