Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and observe patients’ health status by taking vital signs, recording medical documentation, and reporting changes to healthcare professionals promptly.
- Administer medications following home healthcare regulations and medical terminology guidelines while maintaining patient confidentiality in compliance with HIPAA standards.
- Support clients with specialized needs such as dementia care, Alzheimer’s care, memory care, hospice care, and end-of-life assistance.
- Prepare nutritious meals, assist with light housekeeping tasks like cleaning and laundry to ensure a safe and comfortable environment.
- Document patient information accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for seamless communication among healthcare teams.
- Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met with compassion and professionalism.
Skills
- Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments.
- Strong knowledge of medical terminology, medication administration, catheter care, and basic life support (CPR/First Aid).
- Ability to perform patient transfers safely using patient transfer methods and equipment like Hoyer lifts or other mobility aids.
- Excellent patient observation skills to monitor health changes or behavioral cues indicative of conditions such as dementia or Alzheimer’s disease.
- Familiarity with home health agency operations, hospice care protocols, and home & community care standards.
- Effective client communication skills combined with compassionate caregiving techniques tailored to diverse needs.
- Ability to handle heavy lifting safely when assisting clients during transfers or mobility activities.
- Experience working within home health care regulations while maintaining patient confidentiality under HIPAA guidelines.
