Schedule: Monday-Friday 8 AM-5 PM
No Call
Service Area: Corona, Riverside, Moreno Valley, Norco, Eastvale, Jurupa Valley, Perris and Redlands
Requirements
Must have at least 1 year of Home Health experience within the last 3 years.
HCHB charting experience is preferred but not required.
Duties
- Implement and modify individualized care plans under the supervision of licensed physical therapists, focusing on patient-specific goals and needs.
- Deliver expert physiotherapy interventions such as trigger point therapy, mobility exercises, and functional training to promote recovery and pain relief.
- Conduct thorough assessments of patients’ physiology, anatomy, and medical history to inform treatment strategies.
- Assist with discharge planning by evaluating patient progress and coordinating with healthcare teams to ensure seamless transitions from inpatient or outpatient settings to home environments.
- Provide compassionate patient care across various settings including home health visits, skilled nursing facilities, outpatient clinics, and acute care environments.
- Educate patients and caregivers on therapeutic exercises, injury prevention, and self-care techniques to support ongoing health management.
- Maintain accurate documentation of treatment sessions, progress notes, and care plans in compliance with healthcare regulations.
Skills
- Strong knowledge of physiology, anatomy, and medical terminology essential for effective patient assessment and treatment.
- Proven experience in inpatient settings, skilled nursing facilities, outpatient clinics, or home health environments.
- Expertise in physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation.
- Familiarity with discharge planning processes and coordination within multidisciplinary healthcare teams.
- Ability to adapt treatments for diverse populations including geriatrics and pediatrics while maintaining a patient-centered approach.
- Excellent communication skills for educating patients and collaborating with healthcare professionals.
