Job Description – Inpatient Care Manager (Registered Nurse)
Location: Bronx, NY 10451
Department: Utilization Review (Inpatient)
Schedule: 35 Hours/Week | 5 Days/Week | 7-Hour Shifts
Note: This position is for Inpatient (IP) Care Management, not Emergency Department (ED).
Position Summary
Facility is seeking an experienced Inpatient Care Manager (RN) to coordinate patient care throughout the hospitalization continuum. The Care Manager will collaborate with physicians, nursing staff, patients, families, and interdisciplinary teams to facilitate care coordination, utilization review, discharge planning, and ensure appropriate resource utilization while maintaining regulatory compliance and delivering high-quality patient-centered care.
The ideal candidate will possess a strong clinical nursing background with experience in acute care, care coordination, utilization review, and discharge planning.
Key Responsibilities
- Coordinate and manage inpatient care plans throughout the patient's hospitalization.
- Perform comprehensive patient assessments to identify discharge needs and barriers to care.
- Develop individualized discharge plans in collaboration with physicians, nursing staff, social workers, and community resources.
- Conduct utilization review activities to ensure appropriate level of care and medical necessity.
- Coordinate transitions of care to post-acute settings including rehabilitation, skilled nursing facilities, home care, and community resources.
- Monitor patient progress and adjust care plans based on clinical needs.
- Collaborate with interdisciplinary healthcare teams to optimize patient outcomes and length of stay.
- Ensure compliance with hospital policies, CMS guidelines, Joint Commission standards, and regulatory requirements.
- Maintain accurate documentation within the Electronic Medical Record (EMR).
- Communicate effectively with patients, families, insurance providers, and external agencies regarding care planning and discharge needs.
- Participate in multidisciplinary rounds and case conferences.
- Identify opportunities to improve quality, patient flow, and resource utilization.
- Utilize computer systems and healthcare software for documentation, reporting, and case management activities.
- Perform additional duties as assigned.
Required Qualifications
- Bachelor of Science in Nursing (BSN) from an accredited nursing program. (Required for this position.)
- Minimum five (5) years of recent clinical experience as a Registered Nurse.

Preferred Qualifications
- Master of Science in Nursing (MSN).
- Leadership or supervisory experience.
- Case Management experience in an acute care hospital.
- Case Management Certification (CCM or ACM).
- Experience working within large academic medical centers.
Required Licensure & Certifications
- Active New York State Registered Nurse (RN) License.
- Primary Source Verification of RN License through the New York State Education Department (NYSED).
- BLS (Basic Life Support) Certification.
- PRI and Screen Certification (Required at the time of submission).

Pay: $64.00 - $68.00 per hour
