DSW Shift: M-F - 7am to 5pm - New Iberia

We are looking for dedicated individuals to provide support and care for participants in the New Opportunity Waver program, who live independently. As a member of our team, you'll play a crucial role in creating a warm and welcoming environment for the individuals we serve.
Summary of Responsibilities:
- Foster a nurturing atmosphere through meaningful interactions and thoughtful assistance with daily activities.
- Prioritize safety and prevent potential hazards for the individuals in our care.
- Monitor and support the health and well-being of our participants during your shift.
- Collaborate in preparing delicious and nutritious meals as outlined in the care plan.
- Provide assistance with mobility, ensuring individuals are comfortable and secure.
- Providing timely transportation to and from doctor’s visits.
- Assist with personal grooming and dressing to promote a sense of dignity and self-confidence.
- Accompany participants outside the home, offering companionship and support as needed.
- Maintain accurate records of services provided and report any notable changes in condition.
- Administer prescribed medications in accordance with established protocols.
- Mandatory participation in continuous in-service training is expected.
- Upholds the rights and confidentiality of the individuals being served.
Qualification Requirements: Your dedication and compassion are what truly set you apart. Alongside a high school diploma or GED/HiSet, all prospective team members must be 21 years of age or older and must have safe reliable transportation and maintain valid insurance. Your reliable transportation ensures you can continue to be a pillar of support for our residents. Physical Demands: Your hands-on involvement is crucial, and your adaptable nature ensures you're equipped to address the distinct needs of each resident.
