POSITION SUMMARY
Diversity Health Center is actively seeking a Registered Dietitian on a part-time or contractual basis. Under the direction of the Chief Medical Director, incumbent is responsible for the implementation and coordination of health education and outreach services.
JOB RESPONSIBILITIES
- Conduct weekly counseling sessions with patients using an evidence-based and patient-centric approach to care.
- Demonstrate strong clinical and counseling skills, leading with empathy and kindness.
- Help patients get to the root of their health concerns to create sustainable behavior changes rather than taking a short-term fix approach.
- Collaborate with social and community agencies in enhancing program development.
- Develop measurable Health Educator objectives/outcomes for the purpose of program evaluation.
- Provide in-house and off-site classes in the community as needed.
- Maintain a statistical database to accurately quantify services and their quality in meeting established outcome measurements.
- Maintain communication with DHC staff in planning and implementation of the program.
QUALIFICATIONS
Bachelor’s Degree in Nursing or Bachelor’s Degree in Health Education. Minimum 2 – 3 years experience in a primary care setting required. Experience in Health Education program development.
Licensure, Certification, or Registration Requirements for Hire:
Registered as a Dietitian (RD) by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics
Current CPR certification including BLS.
ADDITIONAL QUALIFICATIONS
- Knowledge of medical terminology, HIPAA compliance and basic clinical exam procedures.
- Knowledgeable use of teaching techniques and aids.
- Gather and evaluate statistical data to determine program effectiveness.
- Experience using an Electronic Medical Records or Electronic Health Record Software – preferably eClinicalWorks
- Must have great communication skills and computer skills.
