Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and catheterizations.
- Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
- Provide wound care, including suturing and sterile dressing changes, ensuring infection control standards are maintained.
- Support patients with activities of daily living (ADLs), including mobility assistance using equipment like Hoyer lifts and tube feeding management.
- Document all patient interactions thoroughly within Electronic Health Record (EHR) systems such as Epic or Cerner while maintaining strict compliance with HIPAA regulations.
- Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care strategies to optimize patient outcomes.
Skills
- Proven experience working with diverse patient populations in settings such as long-term care, hospitals, or outpatient clinics.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10) for accurate documentation and billing.
- Skilled in IV insertion, catheter care, phlebotomy, medication administration, and basic life support (BLS/CPR).
- Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for efficient medical records management.
- Experience working with individuals with disabilities or cognitive impairments such as Alzheimer’s disease or dementia; knowledge of memory care best practices.
- Ability to perform triage assessments in urgent or emergency situations within emergency room or urgent care environments.
- Excellent caregiver communication skills to effectively educate patients and families about treatment plans and health coaching strategies.
