Summary of Position: Coordinates care and provides skilled physical therapy to patients in the home setting.
Essential Functions:
- Completes the initial assessment/evaluation visit and regularly reevaluates the client’s care needs during each visit
- Initiates or reviews the plan of care and makes necessary revisions
- Provides therapy care based on established plan of care and physician orders or delegates needed therapy services in the home setting
- Initiates appropriate preventive and restorative therapy procedures
- Prepares clinical and progress notes and summaries of care
- Counsels the client and family in meeting related needs
- Ongoing interdisciplinary assessment of the patient
Qualifications/Requirements:
- Current Maryland Physical Therapy License
- 2-3 years Physical Therapy experience
- Current American Heart Association BLS for healthcare providers
- Prior experience in home care preferred
- Demonstrate competency in applying the principles, methods, materials and equipment used in physical therapy
