Caregiver To You is looking for a dependable, caring, and organized Care Manager to join our home care team. This position plays an important role in supporting our clients, caregivers, and families by helping coordinate care, manage schedules, and make sure services run smoothly. This is a full-time position, with some after-hours on-call responsibilities. Availability may include weekends and holidays as needed, especially for urgent staffing needs, caregiver call-offs, or client concerns. The right person for this role is someone who is calm under pressure, communicates well, and genuinely enjoys helping people. Home care can change quickly, so we are looking for someone who can stay organized, problem-solve, and follow through.
Responsibilities
- Manage client and caregiver schedules to help ensure shifts are properly covered.
- Communicate with clients, families, caregivers, and office staff in a professional and compassionate manner.
- Respond to caregiver call-offs, schedule changes, and urgent staffing needs.
- Assist with client check-ins, service updates, and care-related follow-up.
- Help match caregivers with clients based on care needs, availability, and personality fit.
- Document schedule changes, client updates, caregiver concerns, and important communication.
- Support new client starts, ongoing care coordination, and service changes.
- Help maintain strong relationships with clients, families, and caregivers.
- Participate in after-hours on-call responsibilities as scheduled.
- Be available for occasional weekend or holiday needs depending on client and staffing situations.
- Assist the office team with administrative tasks related to care coordination and staffing.
Qualifications
- Prior experience in home care, caregiving, healthcare, staffing, scheduling, or customer service preferred.
- Strong communication and people skills.
- Comfortable handling phone calls, text messages, and time-sensitive situations.
- Ability to stay calm and professional when schedules change or urgent needs come up.
- Organized, dependable, and able to follow through with tasks.
- Proficient with basic computer programs, including Microsoft Outlook, Excel, Word, and Teams.
- Comfortable learning scheduling software, documenting updates, and maintaining accurate records.
- Must be comfortable working with older adults, families, and caregivers.
- Must be reliable and able to maintain confidentiality.
- Bilingual Spanish is a plus, but not required.

Pay: From $22.00 per hour
License/Certification:
