ACMS Home Health Services, LLC is hiring compassionate, dependable, and experienced Home Health Aides (HHAs) to provide quality home care to patients throughout our service area. Position Summary
The Home Health Aide provides personal care and supportive services to patients under the supervision of a Registered Nurse or licensed therapist in accordance with the physician's Plan of Care and agency policies.
Responsibilities
- Provide personal care including bathing, grooming, dressing, and personal hygiene.
- Assist with toileting and incontinence care.
- Assist patients with walking, transfers, positioning, and mobility.
- Perform range of motion exercises as directed.
- Take and document vital signs when assigned.
- Observe and report changes in the patient's physical or emotional condition to the supervising Registered Nurse.
- Assist with meal preparation, feeding, and hydration.
- Perform light housekeeping related to the patient's care.
- Maintain a clean, safe, and comfortable home environment.
- Document all patient care accurately and on time.
- Follow infection prevention and safety procedures.
- Maintain patient confidentiality in accordance with HIPAA regulations.
- Attend required meetings, training sessions, and continuing education.
- Communicate effectively with patients, families, nurses, therapists, and physicians.
Requirements
- Valid 75 Hour Home Health Aide Training Certificate that meets Medicare requirements or current Florida Certified Nursing Assistant certification eligible to work as a Home Health Aide.
- Current CPR Certification.
- Valid Florida Driver License.
- Reliable transportation.
- Current automobile insurance.
- Ability to pass a Florida Level 2 Background Screening.
- Current physical examination and required health screening documentation.
- Excellent communication and customer service skills.
- Compassionate, dependable, professional, and patient focused.
- Ability to work independently and as part of a multidisciplinary healthcare team.
Preferred Qualifications
- Previous home health experience preferred.
- Experience caring for elderly, disabled, or medically complex patients.
- Bilingual English and Spanish preferred.
- Knowledge of Medicare home health documentation (Kinnser/WellSky) preferred.
