Job Summary
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our Night Operations Center (NOC) team on the shift. In this vital role, you will provide exceptional patient care, monitor patient status, and coordinate clinical activities during overnight hours. Your expertise in critical care, medical documentation, and patient assessment will ensure seamless communication between healthcare teams and optimal patient outcomes. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine immediate needs and ongoing care requirements.
- Utilize EMR (Electronic Medical Record) systems such as Epic or Cerner to accurately document patient information, medical history, medication administration, and clinical notes in real-time.
- Administer medications, injections, IV infusions, and perform catheter care following strict aseptic techniques and HIPAA compliance standards.
- Monitor patients on ventilators or other life support equipment; manage airway interventions and respond promptly to acute changes in patient condition.
- Coordinate with interdisciplinary teams for discharge planning, case management, and post-acute care to ensure continuity of care across settings such as hospitals, nursing homes, assisted living facilities, or home care.
- Conduct triage assessments for incoming patients or calls; prioritize urgent cases such as trauma or infectious disease concerns while providing appropriate guidance.
- Support infection control protocols and participate in quality improvement initiatives related to patient safety and healthcare standards.
Experience
- Valid LVN license with current state licensure required; additional certifications such as Basic Life Support (BLS) are essential.
- Demonstrated experience working in acute care settings such as ICU, emergency room, or hospital medicine; trauma center experience preferred.
- Proven background in critical care environments including Level I or Level II trauma centers or neonatal/pediatric intensive care units (NICU/PICU).
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks is highly desirable.
- Prior experience providing specialized care for patients with Alzheimer’s disease, dementia, memory impairments, or behavioral health needs is advantageous.
- Knowledge of ICD coding (ICD-9/10), medication administration protocols, pain management techniques, and infection control practices is required.
- Experience working with individuals with developmental disabilities or in home & community care settings will be considered a plus.
