Job Summary
The Customized Community Support (CCS) role is designed for compassionate individuals dedicated to enhancing the quality of life for those in need of support within their communities. As a CCS provider, you will work closely with clients to develop personalized plans that cater to their unique needs and goals. Responsibilities
- Collaborate with clients to assess their individual needs and preferences.
- Develop and implement customized support plans that align with clients' goals.
- Provide assistance with daily living activities, including personal care, meal preparation, and household management.
- Facilitate community engagement by connecting clients with local resources and activities.
- Monitor client progress and adjust support plans as necessary to ensure effectiveness.
- Maintain accurate documentation of services provided and client interactions.
- Advocate for clients' rights and needs within the community.
Skills
- Strong interpersonal skills with the ability to build rapport with clients.
- Excellent communication skills, both verbal and written.
- Empathy and patience in dealing with individuals from diverse backgrounds.
- Problem-solving skills to address challenges that may arise during service delivery.
- Organizational skills to manage multiple client cases effectively.
- Experience in social services or related fields is a plus but not required.

Job Types: Full-time, Part-time
Pay: $16.00 - $16.50 per hour
Expected hours: 20 – 40 per week
