Qualifications:
- Current, unrestricted license to practice as a Registered Nurse (RN) in the State of Georgia
- Minimum of two (2) years of nursing experience, preferably in home health, private home care, or a related healthcare setting
- Experience working with elderly individuals, persons with physical or mental disabilities, and veterans preferred
- Applicants must obtain professional liability insurance coverage within sixty (60) days of employment as a condition of continued employment.
Knowledge of:
- Home care service delivery and current methods of home management
- Provision of personal care services as defined under Georgia DCH regulations
- Client assessment, care planning, and ongoing evaluation
- Coordination of care and collaboration with PCAs, CNAs, and agency leadership
- Regulatory documentation and compliance standards
- Ability to meet Georgia DCH background screening requirements
- Current TB clearance per agency policy
- Current CPR and First Aid certification
Education:
Graduation from an accredited school of nursing with the appropriate entry-level degree required to practice as a Registered Professional Nurse
Duties & Responsibilities:
- Perform client assessments during initial face-to-face visits and follow-up visits as assigned
- Assist in the development, implementation, and revision of individualized client service plans under the direction of the Nurse Manager and/or Administrator
- Participate in supervisory and monitoring visits to evaluate the provision of personal care services
- Provide clinical input regarding appropriate staffing based on client needs and acuity
- Complete nursing notes, clinical documentation, and visit records in accordance with Georgia DCH regulations and agency policy
- Observe PCA/CNA service delivery and report concerns, incidents, or changes in client condition to the Nurse Manager or Administrator
- Assist with in-service training and staff competency support as assigned
- Support quality assurance and compliance activities as directed
- Communicate client progress, concerns, or service issues promptly through established reporting channels
- Perform additional clinical duties as assigned by the Nurse Manager or Administrator
