Job Description

We are seeking compassionate Caregivers to join our team and provide exceptional in-home elder care. The ideal candidate will have experience in Caregiving with seniors, home community care, assisted living, direct support, or hospice care.
Our Caregivers provide professional and personalized care to each person supported that requires assistance with daily living activities such as feeding, bathing, dressing, grooming, toileting, etc. Our goal is to ensure that each person supported feels seen, heard, well cared for, and supported in living meaningful lives.
Boulder County Expansion Needs:
As we launch in Boulder County we are especially looking for:
- Established caregivers in the local area, including
- Care Managers, Discharge Planners, CNA's, and other experienced Caregiving professionals who want to help bring a new model of care to Boulder County
- People who want to support seniors with connected compassionate care
- People who want to support caregivers with a living wage, benefits, meaningful training, support groups, dementia education and support, and a vibrant caregiver community.
- Visiting and communicating with referral sources, Doctor’s Offices, Hospital Staff in the community
Duties:
- Establish a reliable, consistent, and trustworthy relationship with the person supported and their near and dear (i.e., their families and friends).
- Actively engage with each person supported through attention, conversation and companionship.
- Provide support with daily living activities, such as bathing, grooming, dressing, eating, exercising, and using the bathroom.
- Identify and assess the needs of each person supported and collaborate with the care team members to develop plans for continuing services. Address said needs with respect and attention to safety and make adjustments, as needed.
- Perform health care-related tasks if appropriate such as taking vital signs and administering medication (including over the counter medication) as prescribed by a medical provider.
- Establish and maintain a safe and healthy environment by following company standards and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards.
- Assist with nutritious meal preparation, planning, and grocery shopping for each person supported according to prescribed diets, preferences, and allergies, as needed.
- Accompany the person supported to medical visits or other trips outside the home, providing transportation, as needed.
- Perform light housekeeping duties, such as cleaning, doing laundry, and washing dishes, as needed.
- Provide emotional support to the person supported by caring for them, preparing healthy meals, living independently, or adapting to disability or illness, if applicable.
Administrative Responsibilities:
- Read and become familiar with the health care plan & notes of each person supported.
- Prepare and maintain records of care, conditions, progress, or changes in the condition of the person supported.
- Monitor and report changes in health status and communicate all pertinent information including observations and/ or concerns to the support liaison.
- Respond to all communications (i.e., phone calls, texts, emails) from the support liaison in a timely manner.
Additional Responsibilities:
- Attend monthly caregiver support group meetings.
- Assist the person supported with caring for their pet(s), if applicable.
- Stay up to date with all emergency procedures in the home such as DNR, CPR, fall risk, escape routes, etc.
- Perform other duties as necessary to care for the person supported.
- Certification in CPR and First Aid is a plus
- Understanding of nurse responsibilities

EDUCATION
- High school diploma or general education degree (GED) preferred
CERTIFICATIONS & LICENSES
- Valid driver's license with a registered and insured vehicle required
- A "satisfactory" driving record required
- CPR Certified preferred
EXPERIENCE
- One to two years of professional Caregiving, family caregiving, or related experience required
- Experience in social services or geriatric related services is a plus
- Must possess a strong service orientation, including active listening, critical thinking skills, and close monitoring of clients.
- Well-versed in first aid and other emergency procedures
- Comfortable in an environment in which clients cannot perform basic life duties.
- Basic understanding of medical terminology and basic concepts of personal care
