RN JOB DESCRIPTION
*Perform comprehensive patient assessments and ongoing evaluations.
*Develop, implement, and update individualized plans of care.
*Administer medications, treatments, and wound care as ordered
*Monitor patient progress and report changes in condition to physicians
*Educate patients and families on disease management, medications, and self-care
*Coordinate care with physicians, therapists, and other healthcare professionals
*Maintain accurate, timely, and compliant clinical documentation
*Provides care using infection control measures that protect both staff and patient/client.
*Assures the continuity of care through delivery of quality patient/client care.
*Provides effective communication to the patient/client, family, team members, and other health care professionals as evidenced by clinical notes, case conferences and communication notes, Completes appropriate documentation in a timely manner.
*Monitors assigned cases to ensure compliance with requirements in third party payor.
* Participates in the Agency's QAPI program.
*Participates in Agency-sponsored in-service training.
*Demonstrates commitment, professional growth and clinical competency.
*Promotes Agency philosophy and administrative policies to ensure quality of care.
* Other duties as assigned.

Pay: $30.00 - $35.00 per hour
