Excelsior Healthcare is a growing, compassionate, and professional primary care clinic seeking a Spanish-speaking Chronic Care Management (CCM) Coordinator to join our team. Your language skills will be essential in supporting our Spanish-speaking patients with chronic health conditions and improving their long-term care outcomes.
Job Title: Chronic Care Management (CCM) Coordinator
Location: Buford, GA (On-site)
Job Type: Full-time
Pay: $16.00 – $19.00 per hour (based on experience)
Expected Hours: 36 – 40 hours per week
Key Responsibilities:
- Make regular phone calls to patients with chronic conditions
- Assist in creating and updating personalized care plans
- Coordinate follow-ups for medications, labs, referrals, and appointments
- Translate and communicate clearly with Spanish-speaking patients
- Accurately document all CCM activities in the MDLand/EMR system
- Collaborate closely with providers and clinical staff
Requirements:
- Spanish and English preferred
- Experience in a healthcare or customer service role preferred
- Familiarity with EMR systems (e.g., eClinical Works / MDLand) is a plus
- Medical assistant or healthcare background preferred, but not required
- Strong communication, organization, and empathy
