Position Summary: The PRP Rehabilitation Specialist position is responsible for the overall development of individual rehabilitation plans for new consumers authorized for PRP services. The Rehabilitation Specialist assures that the needs of consumers are identified, documented and addressed in accordance with COMAR regulations.
Rehabilitation Specialist: Complete Rehabilitation Assessments to identify the rehabilitation needs of the consumer. Implement the rehabilitation activities outlined in the Consumers IRP. This position provides clinical supervision to our direct care and administrative staff. Provide consumer support as needed. Must be a licensed mental health professional with a minimum of 2 years direct care experience working with emotionally disturbed youth. Licensed/Certified in the State of Maryland, communicate effectively both verbally and in writing.
Essential Duties:
- Conduct Initial Assessments & determine whether consumer needs can be met with Psychiatric Rehabilitation Services
- Write consumer (IRP's) and assure service delivery align with the Individual Rehabilitation Plan.
- Conduct monthly supervision
- Collaborate with multi-disciplinary team to ensure consumer goals are met
- Ensure documentation align with rehabilitation goals
- All employees may be required to perform duties outside of their normal responsibilities as needed.
