OCCUPATIONAL THERAPIST – FULL TIME
We are seeking a licensed Occupational Therapist with SNF experience or strong clinical skills ready to thrive in a fast-paced, team-based environment. The ideal candidate is passionate about geriatric care and functional recovery. This role focuses on helping residents regain independence and quality of life through individualized, goal-driven therapy within a skilled nursing setting.
You’ll collaborate closely with nursing, rehab, and medical teams to deliver care that makes a measurable difference in the lives of long-term care and short-stay rehab patients.

QUALIFICATIONS:
- Bachelor’s or Master’s Degree in Occupational Therapy
- Active OT license in the state of operation (required)
- Experience in SNF or geriatric rehab strongly preferred
- Strong documentation and EMR skills
- Knowledge of Medicare Part A & B therapy regulations
- Excellent communication and teamwork abilities
- Adaptable, positive attitude working with older adults
- New grads with SNF fieldwork encouraged to apply
RESPONSIBILITIES:
- Evaluate and treat residents with functional deficits in ADLs, mobility, and cognition
- Develop and implement patient-centered care plans
- Provide skilled interventions to meet short- and long-term rehab goals
- Document daily treatment sessions and progress notes in accordance with SNF standards
- Recommend and train on adaptive equipment as needed
- Collaborate with nursing and interdisciplinary team on discharge planning
- Ensure compliance with CMS, facility, and professional regulations
